How To Qualify for Meals
There are two ways individuals can get access to MANNA’s nutritious meals free of charge:
- Your case manager, dietitian, doctor, nurse practitioner, or physician's assistant needs to complete a referral form and fax it to MANNA at 215-496-9102. To download our referral form, click here.
- Clients can also refer themselves by calling 215-496-2662 x 117. Our client services coordinator will check to see if an individual should be referred to one of our nutritionists, and then coordinate with the healthcare provider to secure the required medical information.
Qualification consideration is based upon nutrition status. Some nutrition indicators include: recent weight changes, abnormal laboratory values, multiple medications, response to medical treatments and many other conditions which can impair nutritional status. After reviewing the referral information, MANNA dietitians coordinate with the client to recommend the appropriate services for the client: nutritional counseling, appropriate meal service, or intensive counseling with meal delivery. MANNA is only able to serve people at nutritional risk due to a life-threatening illness. MANNA clients receive meals for 6 months to start.
Every 6 months, MANNA reviews a client's health and nutrition progress to evaluate eligibility. The medical provider will be asked to partner with the nutrition staff to complete the health update.
